Operations/Forms & Checklists

Paperwork that gets filled out, not put off.

Turn inspections, incident reports, requests, and daily checklists into forms your team completes on a phone or a computer. Photos, signatures, and location come back with every submission.

Twelve field types

Text, numbers, yes or no, dropdowns, dates, times, ratings, signatures, photos, and location. Build the form you already use on paper.

Photos and signatures built in

A picture of the finished job and a customer signature are part of the form, not a separate text message.

Submitted without signal

Forms save on the device and upload when the phone reconnects. Nobody retypes a report.

Build on the web. Fill on the phone.

Built in minutes. Filled in seconds.

Drag the form together from twelve field types, add a rule for what happens on a No, and publish it to a location or a team. The people who fill it out get a clean form on their phone, with photos, signatures, and location built in, and it works without a signal.

Building a form

Drag, set the rules, publish. No paper to print.

  • Twelve field types, with sections for long forms
  • Required fields, help text, and default answers
  • Rules: show, hide, or require a field based on an answer
  • Publish to a location, a team, or a role
  • Repeat a checklist by day, time, and location
  • Templates for inspections, incident reports, vehicle checks, and opening and closing

Filling one out

On the phone or the web, in the middle of the work.

  • Photos from the camera, right in the form
  • A signature drawn with a finger
  • Location and time stamped on the submission
  • Saves as you go, so nothing is lost mid-form
  • Works with no signal and uploads when it returns
  • Submit once, and the right manager gets it

The twelve building blocks of every form.

  • Text
  • Number
  • Yes / No
  • Multiple choice
  • Dropdown
  • Rating
  • Date
  • Time
  • Photo
  • Signature
  • Location

Rules

Plain-language rules on any field. No code.

  • Required
  • Show or hide a field
  • Require a photo on a No
  • Require a note on a No
  • Notify a manager
  • Create a task
  • Score the form

After submit

A submission is the start of the follow-up, not the end.

  • Manager review
  • Status and comments
  • Task from a failed item
  • PDF of every submission
  • Export to CSV
  • Kept with the location's history

Moments in a form

From a blank form to a finished report.

Twelve field types, rules that react to an answer, forms that work without a signal, follow-ups that create themselves, checklists that repeat, and every submission in one list.

Twelve field types

Text, numbers, yes or no, choices, dropdowns, ratings, dates, times, photos, signatures, location, and sections. Enough to rebuild any paper form.

Rules that react to an answer

A No can require a photo and a note, show more questions, notify a manager, or create a task. Set it once on the field.

Works without a signal

In a garage or a basement, the form saves on the phone and uploads on its own when the signal returns. Nobody retypes a report.

A failed item becomes a task

When an inspection fails an item, a task is created for the right person with the photo and note attached, and the manager is notified.

Checklists that repeat

Opening, closing, and daily checks repeat by day, time, and location, and you can see at a glance which days were done.

Every submission in one place

Filter by form, person, place, or date. Review, comment, and mark as reviewed. Export to CSV, or open any submission as a PDF.

Forms & Checklists questions.

Can we recreate the paper forms we use today?

In most cases, yes. Forms are built from individual fields, so you can match the layout and questions your team already knows.

Can a form be assigned to a location or a shift?

Yes. Publish a form to a location, a team, or a role, and set it to repeat by day and time. The people on that location see it on their phone when it is due.

What happens when an item fails an inspection?

Whatever the rule on that field says: require a photo and a note, notify a manager, or create a task for the right person with the photo attached. The submission is flagged in the list until it is reviewed.

Does it work without a signal?

Yes. The form is saved on the phone as it is filled out and uploads on its own when the phone reconnects. The submission keeps the time and location from when it was completed.

Can we export submissions?

Yes. Export a form's submissions to CSV, and open any single submission as a PDF with its photos and signature.
